What To Expect

Brief Phone Intake

After completing the on-line form expressing interest in treatment, you will be contacted to schedule a 20 to 30 minute phone intake in order to determine treatment needs.  The phone intake is not a confirmation that you will receive treatment, but a process of determining whether your treatment needs can be sufficiently addressed at Canvas.

Intake Form

You will be asked to complete an intake form prior to your initial session.  This form will ask you to provide demographic information, medical history, psychiatric history, current relationship status, work/school satisfaction, and several open-ended questions that help me better understand how you view yourself and goals for therapy prior to our first scheduled session.  It is kind of sneak peak to who you are before we meet.

Extended Intake

Think of the extended intake as a getting to know you process.  The purpose is to explore aspects of your history and current functioning reported in the intake.  Your responses will help to guide development of a treatment plan and will be used as a guide directing the first 3 to 4 sessions.

Appointment Scheduling

If you are new to therapy, you are scheduled for weekly individual therapy visit.  However, as symptoms decrease and therapeutic goals have been addressed, some clients benefit from decreasing frequency of visits to biweekly, every 3 weeks or 1 x a month.  Every once in a while, a check in or maintenance session may be provided after therapy has terminated.

Appointments are scheduled after each visit.  You will receive appointment reminders 36 hours before your scheduled appointment.

Cancellation Policy

Please cancel at least 24 hours before your scheduled visit in order to avoid paying the full rate of service.

Lateness

If you are more than 15 minutes late to an appointment, generally there is not much flexibility in extending your appointment to accommodate the 45 to 50 minutes needed.  If accommodations cannot be made, the appointment will be cancelled and we will have to re-schedule.
